ABOUT US
Transitions care ( West Midlands) was created in 2016 with one aim in mind, this is to offer a high level of support to younger adults with profound learning disabilities, autism and behaviors that may challenge services to lead safe, active and fulfilling lives.

We work towards providing the support, encouragement and empowerment that young people need to live fulfilling lives.
​
Our premises are custom built to accommodate the needs of our clients so for us, our unique definition of 'personalisation' means the built environment is personalised to the needs of young adults in addition to the care and support we provide.
​
We use a range of digital technologies to enable support and personalisation and ensure our staff have the right skills to provide the level of quality needed to support young adults with complex needs.
​
All the staff working within our organisation go through a rigorous selection process, and are expected to complete care certification training. This is at a minimum of diploma level 3, and they all also receive special training in personal safety (NFPS) and epilepsy.
​
We strive to continually improve our services and provide the right care that improves the lives of our young adults and enriches their day to day lives whilst providing a supportive environment for our staff.

OUR MANAGEMENT

Philip Brown
Service Manager and Nominated individual
Phil Brown has worked in the care sector for 15 years. He started his career working for Young Options supporting young people with emotional and behavioral difficulties. He then went on to work in a Walsall council managing a home supporting young people with emotional and behavioral difficulties. After 9 years Philip returned to the private sector where he was instrumental in developing services for both children and adult services in the Birmingham area, this included developing two new six bedded services for adults that have moderate to severe learning difficulties.

Russell Davies
Manager of Duel Services
Russell Davies has worked professionally in the care sector for over 22 years. In this time he has worked for Mencap, where he supported adults with profound learning disabilities and behaviors that challenge. He then went on to deputise at Vision Homes association for 4 years, supporting adults with learning disabilities, visual impairment and behaviors that challenge. Russell furthered his skills and experience within the childcare sector, spending 7 year supporting and managing services for young people with emotional and behavioral difficulties. Recently Russell developed and managed a 10 bedded residential service for adult with enduring mental health concerns
